First Visit to Kenneth C. Sprechman, DDS, PA

Your initial appointment will consist of a thorough examination followed by an explanation of  your diagnosis and treatment options. Often treatment can be completed on the same day as the consultation. However, a complex medical history or treatment plan may require an evaluation first and a second appointment to provide treatment on another day.

Please assist us by providing the following information at the time of your consultation:

  • Your referral slip and any X-rays if applicable.
  • A list of medications you are presently taking.
  • If you have dental insurance, bring your insurance card. This will save time and allow us to help process your claim.
  • Please complete the Patient Registration form on this website and submit online or print a copy and bring the completed forms with you.

IMPORTANT: A parent or guardian must accompany all patients under 18 at the consultation visit.

Medical

Please alert the office if you have a medical condition orare taking  medication that may be of concern prior to treatment (i.e. artificial heart valves, artificial joints, rheumatic fever, bisphophonates/ osteoporosis medications, etc.) or if your physician or dentist has advised you to premedicate with antibiotics prior to dental treatment.

Osteoporosis medication

If you are taking medication for Osteoporosis, please read the following important information regarding how this may affect your dental treatment. ADA article “Osteoporosis medications and the risk of developing jaw complications.”  In addition, it is very important that you inform us of any Bisphosphonate medication you are taking.

X-Rays

If your dentist has taken recent x-rays feel free to bring them with you to your appointment. Additional x-rays will be taken using our digital imaging system which requires less radiation than standard x-rays.
